Pfizer Covid-19 Vaccines Available

From mid-January 2022 Medicine On Blackwood will be running Pfizer Covid-19 vaccination clinics.

Please phone reception on 07 3855 9888 or book an appointment online.

Prior to your appointment you will need to complete and sign a consent form.

You can download the consent form for anyone 12 years old or older HERE and the consent form for children aged between 5 and 11 HERE.

If you answer YES to any of the questions on the consent form or if you have any concerns about being vaccinated, you need to make an appointment to discuss these concerns with your regular GP to ensure you are suitable to receive the Covid-19 vaccine.
Your appointment is for the vaccination only and not to discuss suitability.

Please ensure you arrive at the appointment on time and you wear appropriate clothing, such as sleeveless or sleeve pull up shirts.
